What happens after I request my Free Case Review?

We follow up by phone or text to arrange your Free Case Review. Share your goals and questions; together we discuss whether a full PT evaluation, Strong Bones Strength Class, virtual coaching or another next step fits.

Do you bill insurance?

We are a cash-pay practice and do not bill insurance directly. We provide a superbill for possible out-of-network reimbursement and accept HSA/FSA cards for eligible physical therapy expenses. Ask your plan about coverage before your visit. Read our payment and insurance guide.

How long is my first appointment?

Your first individual PT appointment is 60 minutes and includes your assessment and starting plan. The Free Case Review is a separate conversation about your goals and whether care fits; it does not replace an examination.

What should I confirm before my visit?

Confirm the date, time, location or video instructions, appointment length, and any questions about payment arrangements. Ask about parking, building entry or accessibility if you need help planning your visit.

For referring clinicians and care teams

Mindful Movement PT offers one-on-one rehabilitation with Dr. Emily Warren, DPT, Cert. MDT, at 4890 Highland Dr, Holladay, UT 84117, serving Greater Salt Lake City. The first individual PT assessment is 60 minutes.

When an MMPT assessment may fit

Nonurgent back or neck symptoms that limit daily activity; radiating symptoms appropriate for conservative care; difficulty progressing after an earlier exercise plan; and rehabilitation alongside a treating clinician's plan. The assessment connects history, relevant neurological findings, movement response and an activity goal. McKenzie assessment guides exercise selection when appropriate; it is not a standard extension routine for every patient.

How to connect someone with us

  1. Discuss fit or coordinate a handoff: call (385) 332-4939. Ask how to share relevant records securely and arrange communication with Dr. Emily.
  2. Let the patient choose: share this page and invite them to request a Free Case Review. No referral is required to contact us or begin local PT.
  3. Prepare for the assessment: relevant findings, restrictions, previous treatment response and the patient's activity goal help focus the visit. With the patient's permission, discuss how progress or concerns should be communicated to the treating team.

Please do not upload or paste patient records into the public form or ordinary email. Call to arrange the appropriate handoff. Urgent neurological changes and other red flags require medical evaluation rather than a routine case-review request.

Payment: MMPT is cash-pay and does not bill insurance directly. A superbill is available for possible out-of-network reimbursement; coverage is not guaranteed. Confirm the patient's plan and preferences before arranging care.
